Coordinating Care

Navigating the complex world of funding and community-based services can be tough. Disability Supports case managers are here to help. Case managers serve as personal envoys to help coordinate the support and services that are best for clients today and well into the future.

Case Management Services include:

- Coordinating and tracking long-term medical issues and ensuring clients and caregivers follow up with doctors as necessary;

- Helping to complete and process paperwork:  DCF renewals, HUD forms and other reporting documents;

- Creating person-centered support and positive behavior suppport plans, and coordinating all the services for each individual’s objectives.
